Premier League champions Chelsea will get their hands on the trophy following their final day clash with relegated Sunderland on Sunday.

Antonio Conte’s men secured their fifth title in 12 years by narrowly beating West Brom 1-0 last Friday, before returning to Stamford Bridge as champions in a 4-3 success over Watford on Monday night.

And now they will have the chance to really put on a show against David Moyes’ Sunderland, who will be playing Championship football next season after a frankly disastrous campaign this time round.

The Black Cats have managed just six wins from their 37 Premier League games this term, and have only won three of those from their 18 matches on the road.

Clearly, Conte’s Chelsea will be hot favourites to record a 30th win of the campaign on Sunday, which would be a Premier League record, yet the Italian may look to make a few changes to his starting line-up.
