Introduced from the bench at half-time and trailing 2-0, Hazard changed the game with the type of performance which saw him crowned PFA Player of the Year last season.
With seven minutes remaining, the 25-year-old curled a perfect shot into the top right corner for his third goal in his last two league outings, which helped Leicester City eventually be crowned champions.
